			<content:encoded><![CDATA[<p>In the first of these articles; <a href="http://www.rehabcoach.com/blog/2011/09/empowering-e-patients-why-treatment-plans-are-important/">Empowering e-Patients: Why Treatment Plans are Important</a>, a Participatory Model of healthcare and the concept of personal treatment plans was introduced.</p>
<p>Just to recap, a treatment plan is a tool by which the patient obtains the best in terms of their care and ultimately achieves their health related goals. It is key to the concepts of e-Patient and online health management.Â  A treatment plan is very similar to devising an action plan to help achieve career or other life goals. Individuals using a treatment plan will improve their chances of success and so can pursue their health and lifestyle goals during the course of their treatment.</p>
<p>As with any project, the treatment plan will contains milestones and track an individualâ..s progress towards achieving their defined health and lifestyle goals. Â Â Â Again similar to any other project, the fundamental basis to the success of any treatment plan is the power of collaboration which in this case, is between the e-Patient and their treatment team. This is what this second article is going to explore.</p>
<p><strong>So what is this Collaboration?</strong></p>
<p>There are four aspects to achieving an effective Healthcare Collaboration</p>
<ul>
<li>Firstly there must be mutual respect between the group and this is a key success factor for any fruitful collaboration. The collaboration is more likely to succeed when everyone involves shows consideration and respect for the othersâ.. points of view. Â However the e-Patient must respect his support team in the same way they show respect for him and his rights. This follows the â..<strong>give</strong><strong> allÂ model</strong>â. of collaborative respect which occurs when individuals or groups offer each mutual respect.</li>
<li>Secondly, there must be trust between an e-Patient and his treatment team. The e-Patient needs to be confident that his team is trained, experienced and that they have his best interests at heart.Â  Likewise, the treatment team must trust the e-Patient and accept their contributions as being just as worthwhile and important as the â..expertsâ..</li>
<li>Thirdly there must be good communication.Â  Â For example, a doctor must describe medical terms using simple, plain English terms, however the patient should still perform their own online research so that they understand any medical terms and are able to contribute knowledgeably to any conversation.</li>
<li>Finally in order to be able to collaborate, the team must find their common ground and shared values. Â By using social media utilities, and the interaction within them, common ground and shared values can be developed for participating e-Patients.Â  This can be a form similar to traditional social interaction; which in plain words is others observing that you look sick, appear pale or are showing some improvement. In the past this has been the foundation for healing going back many thousands of years.</li>
</ul>
<p><strong>So how does this Collaboration work for an individual?</strong></p>
<p>Letâ..s take John, whoâ..s a line manager at a manufacturing firm. He suffered from lower back pain for the past couple of years and because he spends a lot of time on his feet at work, he really needed to get his <strong>chronic pain </strong>sorted out or at the very least obtain effective <strong>pain relief</strong>.Â  Â In this example, Johnâ..s treatment team could potentially consist of his doctor, his wife, a massage therapist andÂ close friends offering support.Â Â  However the key role in any treatment team is the facilitator.</p>
<p><strong>Role of the Facilitator or Treatment Coach</strong></p>
<p>There are many key tasks that the facilitator will perform. They will assist in defining the goals in the treatment plan. In this example John would set targets for himself and discuss these with his facilitator; the facilitator could then also propose targets for John.Â Â  The facilitator will also direct and assist with achieving these goals. In simple language this would be locating relevant health specialists, arranging medical tests and any other networking or professional support required. Â Â The facilitator will assist John in defining these goals logically, and organizing any necessary steps to be able to realise them.Â  John may share his goals and the treatment plan with his doctor, and likewise John can show any doctor recommendations to his facilitator.</p>
<p>This facilitator may possibly be a trained nurse or doctor but they will be the person who serves as the first point of contact for the e-Patient when receiving treatment and starting any necessary collaboration. Â However, virtual healthcare providers such as <a href="http://www.rehabcoach.com/hello.do">RehabCoach</a> will provide what is called a Treatment Coach. In this example, a Treatment Coach served as Johnâ..s intermediary throughout his course of treatment and collaboration. He first got started by going online, meeting the Treatment Coach, and coming up with a few goals to get his treatment on the way there and then. The keystone to the collaboration is the treatment plan, as the plan is the measure of the quality of the collaboration. For John, his treatment plan served as the focal point for all of the decision making and collaboration that has taken place. His treatment plan became the place to link his goals to actions, as well as being the common ground for defining and sharing goals with the rest of the treatment team.</p>
<p><strong>How is the progress measured?</strong></p>
<p>Once goals are defined and shared, there needs to be a way to measure Johnâ..s progress toward his goals. This is where what is referred to as a treatment score comes in. The treatment score was used to measure Johnâ..s progress toward his health and lifestyle goals by taking into account what heâ..s has achieved since the initial pain assessment.</p>
<p>Through collaboration, John was able to develop a treatment plan that worked and was able to measure any progress he is made toward his stated health goals.</p>
<p><strong>Whatâ..s happening now?</strong></p>
<p>Well John is now making good progress with his <strong>pain management</strong> and the <strong>chronic pain </strong>in his lower back is improving. This is thanks to his treatment plan, his treatment coach and the support from his team. To conclude, John would never have been able to collaborate with health professionals in the conventional healthcare system in the same successful way that he has with this new participatory healthcare model.</p>

No longer do patients have to see a medical practitioner at the hospital, or solely rely on the doctorâ..s advice as the most up-to-date, accurate health information thatâ..s out there. Given the near ubiquity of the Internet, patients are now exercising personal responsibility, leveraging social media, and technology to access health information and to participate in the treatment planning process. This is the new model of healthcare, one that combines personal responsibility with collective intelligence and a participatory network of patients, treatment coaches, and medical practitioners.</p>
<p><strong>The e-Patient</strong></p>
<p>An e-patient is a tech-savvy health consumer who wants to make the shift from being a passenger of healthcare to a responsible driver of their health and their treatment.Â  This person doesnâ..t just want to receive healthcare, but engage their healthcare with medical professionals by being informed. Through the use of the Internet, the e-patient is now knowledgeable about his or her medical condition and is better able to communicate and negotiate with their medical team. An engaged e-patient benefits in two ways: better health choices and medical care, and better relationships with medical professionals. I believe that as online tools become more sophisticated e-patients will take more and more responsibility for their health and define treatment goals on their terms.Â  Consequently, the treatment planning process is shifting to a collaborative one, requiring that medical practitioners work with patients to make decisions and define treatment goals.</p>
<p><strong>The New Participartory Model</strong></p>
<p>As healthcare shifts towards this participartory model the goal of the e-patient will be to optimize their health and lifestyle goals while receiving treatment. The treatment plan becomes the the tool by which the patient optimizes their care, communication and outcomes. The process of negotiation and collaborationÂ will require several changes not required by the traditional healthcare model. These changes include:</p>
<ul>
<li>A chance to build trust through mutual understanding</li>
<li>Transparency â.. Patient and treatment team can hold each other accountable for action (or lack thereof) when both sides collaborate in developing the treatment plan.</li>
<li>Progress Tracking â.. With collaboration and a treatment plan, itâ..s much easier to keep track of the progress you are making as a patient in treatment and in reaching health and lifestyle goals.</li>
<li>Timeliness â.. A treatment plan puts your medical progress on a schedule. There are some health problems, such as chronic pain, that patients cannot afford to have untreated for significant periods of time.</li>
<li>Social Interaction â.. With the increase in prominence of social media, online interactions demand that both parties share the treatment plan and gather additional opinions.</li>
</ul>
<p><strong>How to Use a Treatment Plan</strong></p>
<p>With a treatment plan, the e-patient becomes proactive in aligning their treatments with their lifestyle goals. Â This is because the e-patient will find it easier to collaborate with practitioners to refine and adapt their goals. Much like how documenting career or education goals increases the chances of fulfillment, a treatment plan acts in the same way with health goals. Thus, the treatment plan becomes a critical tool for any e-patient, as it is the key to aligning treatment goals with the e-patient&#8217;s own health and lifestyle goals. By making better choices and e-patients benefits in the following four ways:</p>

			<content:encoded><![CDATA[<p>There is no doubt that <a href="http://www.rehabcoach.com/">chronic pain</a> is a major problem for a large percentage of Americans. <a href="http://www.examiner.com/health-in-national/76-5-million-americans-are-affected-by-chronic-pain">According to an article by Examiner.com</a>, 76.5 million Americans suffer from chronic pain to the extent that it significantly affects their everyday life. But more interestingly, a startling percentage of the 76.5 million chronic pain sufferers are children. <a href="http://www.oregonlive.com/health/index.ssf/2010/04/too_many_kids_live_in_a_world.html">Another study</a> shows that nearly 1 in 12 children in America suffer from chronic pain.</p>
<p>Of the many chronic pain conditions found in children, none requires more hour-by-hour treatment and monitoring than Scoloiosis.</p>
<p>While Scoliosis is not a widespread <a href="http://www.rehabcoach.com/">chronic pain</a> condition in children, it does affect a relatively high percentage. A recent study conducted by the National Association of Arthritis and Muskoskeletal and Skin Diseases (<a href="http://www.niams.nih.gov/Health_Info/Scoliosis/default.asp">NAIMS</a>), found that Ã¢â‚¬Å“of every 1,000 children, 3 to 5 develop spinal curves that are considered large enough to need treatment.Ã¢â‚¬Â And the forms of treatment needed to effectively minimize the chronic pain and mobility issues associated with Scoliosis are wide-ranging enough to make it difficult for parents of children to manage, especially when school is in session.</p>

The idea made me think about how fibromyalgia patients would utilize this, as certain foods can be good, or bad, for fibromyalgia pain. How could Super Marmite help them, and other e-patients?</p>
<p>LetÃ¢â‚¬â„¢s take Lucille, a hypothetical financial analyst whoÃ¢â‚¬â„¢s very busy with her career in the heart of Paris. SheÃ¢â‚¬â„¢s in need of fibromyalgia pain relief so she can continue working hard and progressing her career. Lucille has done some research on fibromyalgia and foods, but doesnÃ¢â‚¬â„¢t have much time to experiment or to prepare meals for herself.</p>
<p>So, Lucille utilizes Super Marmite to find home cooked meals and to learn about which foods provide her pain relief and which donÃ¢â‚¬â„¢t. 			<content:encoded><![CDATA[<p>The trend towards using social networks to follow the day-to-day concerns of those who matter to us, begs the question.  How can social networks help us when we are managing chronic pain?  Jessie Gruman Ph.D. the president of the Center of Advancing Health, the author of several books about health policy, has first hand experience. She is a cancer patient.</p>
<p>Upon reflection of her experiences, she advocates for a step-by-step process of chronic pain management when in those Ã¢â‚¬Å“hard placesÃ¢â‚¬Â, where minds are clouded with intense pain and emotion. When the minds of those in need of pain relief and physical therapy are not thinking straight, they may not always be rational, informed patients to make important health care decisions.  It is under these conditions that social networks are best suited to help those manage the varied and complex group centered ambient interactions required to react too, anticipate and prevent pain.</p>
<p>For example, the decision to take pain medications for carpel tunnel relief, or lower back pain treatment, requires answers to several key questions: How much medication should I take per day? When should I start? When should I stop? How will I know if I am addicted to my medication? Answering these questions may need a social network in order to promote and to ensure shared and informed decision making for every step of chronic pain treatment.</p>
<p>A shared participation network, according to Gruman, embodies several values that ought to be held by all involved in this shared participation network, such as physicians, treatment coaches, even other patients suffering from chronic pain. Values include:<br />
Ã¢â‚¬Â¢Patients and caregivers understanding evidence, using it to help them weigh their options, discussing it with their providers, and their providers doing the same in return.<br />
Ã¢â‚¬Â¢Patients are explicitly invited to participate in the decisions about their care (even if that participation consists of delegating decision-making to a caregiver or physician).<br />
Ã¢â‚¬Â¢The opinions and preferences of patients Ã¢â‚¬â€œ informed by their understanding of the evidence Ã¢â‚¬â€œshape and determine the tests they take and the treatment they undergo.</p>
<p>Compounding the complexity of managing pain is the fact that people with chronic pain are twice as likely to suffer from depression and anxiety than those without. So itÃ¢â‚¬â„¢s a chicken or the egg type of question.  Are the psychological issues the causal factor in the Ã¢â‚¬Å“chronic natureÃ¢â‚¬Â of the pain, is the for example Ã¢â‚¬Å“depressionÃ¢â‚¬Â a result of the pain, or is it a vicious circle?  Either way, the ability to regularly check-in with others to help nudge you into making the right choices is the best strategy for maintaining treatment continuity and consistency.</p>
<p>Why go through pain management on your own while having mental health issues? A social network involving shared participation in this process is an excellent way to overcome the obstacle of making a good decision while in pain, in getting positive feedback about decisions and treatment, and in addressing the issues surrounding the use of pain medications as treatment.<br />
Utilizing a social network to find a treatment that affects all aspect of life and well-being can ensure that shared, participated decisions are made about pain relief and pain medications.</p>

<p>Many who suffer from chronic pain have a hard time determining the  what, when and how of getting the proper support.Â  Pain suffers make the  common mistake of solely relying on their physicians and therapists to  get the support they need.Â  While doctors and therapist should always be  an important component of a personâ..s support network, pain suffers must  also explore other options.Â  Other support venues outside of the  medical practice include friends and family, online forums, neighborhood  support groups, and social networking groups (facebook, myspace).Â   While varied, the level and appropriateness of the support is often  inadequate.</p>
<p>For instance, with online support forums, a member may respond to  your question, but he or she might not be online tomorrow or next week.  So if you have issues that require consistent long term support, you may  have to post your question again and hope for the best.</p>
<p>So how does a pain suffer determine what is right for them? The key  criteria any pain sufferer should consider when seeking support is time,  urgency and complexity.</p>
<ul>
<li>How much time do they have to      invest to get the support that they need?</li>
<li>To what degree is the pain      debilitating and how urgently must it be addressed?</li>
<li>And finally, is the treatment      resolution simple or complex?</li>
</ul>
<p>Some of the above issues can require individualized dedicated  support, while others can be addressed with a single answer. In  addition, depending on the complexity of the issue, the duration of the  support may require one day to one month too resolve. This means that  you must know which support option (message board, doctor, friends,  social networking etc.) can best address your needs.</p>
<p>Given the hectic nature and multiple demands of every day life  getting the right level of support can be a daunting and frustrating  task.Â  As a result, the best you can hope for from traditional support  channels is that you receive â..ad-hoc supportâ. that isnâ..t outcome  focused, and relies on personal preferences.</p>
<p>To address the varied and daily concerns of pain sufferers a new category of pain support called the pain support system  has been developed. Â Pain support systems provide a consistent level of  support to address most of the issues of pain sufferers must confront.
